Thanks to state-of-the-art technology some VNA, Western Pennsylvania patients are now getting a check-up every day in their own homes. VNA, Western Pennsylvania is the exclusive provider of the Honeywell HomMed® system to collect patients’ vital signs on a daily basis. With this system, VNA, Western PA puts hospital-grade monitors in patients’ homes to gather clinical data regarding trends in their health status.

Honeywell HomMed® monitoring units are individually programmed for patients according to parameters established by their personal physician. The units are small, about the size of a clock radio, and simple to use. Our technicians will set up the unit and teach the patient how to use it. Data is collected each day and transmitted to the VNA, Western PA where a trained individual reviews the results. Reports can be faxed directly to the patients’ physicians.

How Does It Work?
Honeywell HomMed® units are small. They take up about as much space as a clock radio. A technician from the VNA will setup the unit in the patient’s home and provide training in its use. The unit requires a standard electrical outlet and a telephone jack. The Honeywell HomMed® unit is programmed with whatever settings and health questions the physician recommends.

At the same time each day, a voice prompt will instruct the patient to take vital signs, such as weight, blood pressure, pulse, oxygen saturation and temperature. The voice guides the patient step-by-step through the process of pushing one of three color-coded buttons that enable the system to collect vital signs data.

Following the collection of vital signs the system will ask a series of questions that the physician has provided. It takes only 3 minutes a day, and it is easy to do!

Once all data has been collected the Honeywell HomMed® system transmits the data to a clinician at VNA, Western PA who reviews the data. If there is anything out of the ordinary in the readings, the clinician will call the patient. If any vital signs fall outside of the readings the physician has recommended, the unit will signal our clinicians and alert them to the patient’s condition. The problem can then be identified and treated before it becomes serious.

VNA, Western PA will keep track of the patient’s readings and can provide physicians with weekly or monthly graphs of each patient’s vital signs. These reports allow physicians to see subtle changes in conditions and schedule follow-up office visits accordingly.

Patients who use the Honeywell HomMed® monitoring system often have fewer visits to the emergency room and can stay out of the hospital - in their homes where they are most comfortable.

Even if the patient is not currently under a physician’s care, the Honeywell HomMed® system can provide a sense of security that comes from knowing that vital signs are being monitored on a daily basis.

The system is not meant to replace, but rather to complement visits by the VNA, Western PA home care professional. The Honeywell HomMed® system allows them to monitor how the patient has been doing on a daily basis even if they aren’t visiting the home every day. The Honeywell HomMed® system is not a substitute or replacement for HealthWatch™, 911 or other emergency systems. If an emergency arises, the Honeywell HomMed® system cannot signal paramedics, firefighters or police for immediate help.
