How do I Refer a Patient to VNA?

If you have a patient whom you believe would benefit from VNA’s services, we’ve made the referral process easier than ever. Simply complete and submit our online referral form, and in most cases, VNA can initiate services within 24 hours of receiving a referral.

If you prefer, you may also make a referral by calling our central intake department at
1-877-862-6659 or downloading a PDF of our referral form and faxing to 724-282-0981.

If you are uncertain whether your patient qualifies for in-home care, VNA will perform a

complimentary home evaluation to determine eligibility. General criteria for services include:

Skilled nursing
Physical therapy
Occupational therapy
Speech therapy
Social work services
Home health aide services (only when another qualifying skilled service is needed)

Patient must have physician order
Patients must be homebound in most cases; this is mandatory for Medicare patients
Patient must have a need for skilled intermittent care such as:
