VNA utilizes Honeywell's HomMed® technology to provide the best quality in-home care for cardiac patients and peace of mind for caregivers in knowing that their loved one's vital signs are being monitored on a daily basis.

The hospital-grade device, which is no larger than a clock radio, uses voice prompts and visual cues to help the patient measure weight, temperature, blood pressure, blood oxygen saturation and pulse. To get a more comprehensive assessment of the patient's well-being, the device can also ask up to ten health-related questions.

HomMed® then transmits this data to the patient's VNA health care team, who analyze and closely monitor the information received for any indication of changes in the patient's medical condition.

This technology is especially important for patients with congestive heart failure, since they are particularly susceptible to the need for frequent hospitalization as the result of fluid build-up in the lungs, tissue swelling and shortness of breath. Because our nurses are able to take a proactive approach to managing the patient's symptoms at home, emergency room visits and hospitalizations can often be avoided through early detection and prompt intervention that keeps potential problems from escalating.

VNA also provides extensive education for patients with CHF, encouraging them to understand the disease, take their prescribed medications, follow their recommended diet and make necessary lifestyle changes. The HomMed® system plays a key role in this educational process as well, as it allows the patient to see the cause and effect of diet and medication on their condition.
