How Do I Get Services?
If you or a loved one needs in-home care, hospice services or medical equipment, the first step is to discuss this option with your physician, social worker or hospital discharge coordinator and obtain a referral.
Be aware that you have the right to select the home care provider of your choice, and that if you do not specify a particular agency, the doctor or social worker will select one for you. In the case of a hospitalization, it is important prior to being released from the hospital to verify that a home care order has been written notating the agency you have requested.
In most cases, VNA can initiate services within 24 hours of receiving a referral, unless otherwise specified.
