In-Home Hospice
When a life-limiting illness no longer responds to curative treatment, VNA offers hospice care focused on providing comfort, hope and support for patients while allowing them to spend their final days at home.
Our skilled nurses and specially trained volunteers work together to provide the best physical, emotional and spiritual care, not only for the patient but for their loved ones as well.
Our interdisciplinary team of oncology clinical nurse specialists, hospice nurses, social workers, counselors and home health aides collaborate with physicians to implement a personalized plan of care that is individualized to their specific needs and includes:
- Education about the illness, the plan of care and reducing complications
- Pain and symptom management
- Assistance with personal care needs
- Medical equipment and supplies as needed
- Identification of community resources
- Development of personal and community support networks
- Counseling for patient and family
Hospice visits are scheduled to meet the ever-changing needs of the patient and their family. Between visits, hospice nurses are on call 24 hours a day, seven days a week to assist with questions and concerns.
