How Do I Refer a Patient?

If you have a patient whom you believe would benefit from VNA’s services or who has requested a referral to VNA, please call Central Intake at 724-282-6806 or complete our referral form and fax to 724-282-0981. In most cases, VNA can initiate services within 24 hours of receiving a referral, unless otherwise specified.

If you are uncertain whether your patient qualifies for in-home care, VNA will perform a home evaluation visit to determine eligibility. General criteria for services include:

  • Patient must have physician order
  • Patients must be homebound in most cases; this is mandatory for Medicare patients
  • Patient must have a need for skilled intermittent care such as: 
  • Skilled nursing
  • Physical therapy
  • Occupational therapy
  • Speech therapy
  • Social work services
  • Home health aide services (only when another qualifying skilled service is needed)
