VNA utilizes Honeywell’s HomMed® system to provide the best in-home quality care for patients and peace of mind for caregivers in knowing that their loved one’s vital signs are being monitored on a daily basis.
The hospital-grade device, which is no larger than a clock radio, uses voice pro mpts and visual cues to help the patient measure weight, temperature, blood pressure, blood oxygen saturation and pulse. To get a more comprehensive assessment of the patient’s well-being, the device can also ask up to ten health-related questions.
HomMed® then transmits this data to the patient’s VNA health care team, who closely monitor and analyze the information received for any indication of changes in the patient’s medical condition. Should there be anything out of the ordinary in the readings, VNA clinicians will take steps to identify and treat problems before they become serious. VNA will also provide weekly or monthly reports to the patient’s physician so that he or she can track subtle changes in the patient’s health.
The HomMed® system is a great way to supplement visits by VNA home care professionals, as it enables them to monitor the patient’s status on a daily basis, even if they are not visiting the home every day.
