VNA Home Care Services - Diabetes Care

Because diabetes has the potential to have significant impact on a patient’s overall health, VNA offers a diabetes care program that combines patient education and early intervention practices in a proactive approach to managing symptoms at home.

Developed by clinicians, dietitians and certified diabetes educators with the assistance of physicians, our program is designed to help the patient manage their diabetes more effectively day to day in order to prevent potentially serious long-term complications such as heart disease, stroke, amputation, kidney disease, neurological complications, vascular disease and eye disease.

Key to the success of the program is consistent monitoring of the patient’s blood sugar levels by utilizing Honeywell’s HomMed® telemonitoring system in conjunction with the patient’s glucometer. The HomMed® system allows the patient to send blood sugar readings to VNA, where a nurse can review and track this data to identify patterns in blood sugar level changes and prevent problems that might require emergency treatment or hospitalization. In addition to glucose monitoring, the plan of care also encompasses insulin preparation and administration and meal plan management.

As part of the program, a certified diabetes educator visits the patient in their home and helps them understand the disease process, the risk factors and how to modify diet and behavior, providing an extra sense of accountability for the patient. The HomMe® system plays a key role in this educational process as well, as it allows the patient to see the cause and effect of diet and medication on their condition.
