A home health care professional specializing in gerontology, social work and counseling, our certified geriatric care manager conducts an individual assessment of each patient to identify problems, determine eligibility for assistance and evaluate their level of need for services, teaming with the patient and their family to implement the best continuing care program available.These services are particularly beneficial for older adults who need assistance but have no primary caregiver or whose caregiver lives too far away to oversee their care needs on a daily basis.
