VNA Home Care Services - Rehabilitation & Therapy

For many patients who need rehabilitative or therapeutic services, in-home care is a safe, convenient and effective alternative to placement in a rehabilitation hospital or nursing home.

VNA’s interdisciplinary team of physical, occupational and speech therapists, mental health nurses and home health aides work closely with physicians to follow the recommended course of therapy and assess the patient’s progress.

Research has shown that there are many benefits to receiving rehabilitative therapies at home. Remaining in the home is less disruptive to the patient and therefore more conducive to a successful recovery. In addition, allowing the patient to remain independent provides added motivation to follow the prescribed course of therapy and return to optimal levels of functioning. Also, eliminating the need to transport the patient to a physical therapist’s office several times a week reduces the risk of injury.

Additionally, since the patient sees the same team of nurses or therapists regularly, subtle changes are more likely to be noticed and detrimental factors in the patient’s environment or behavior, such as lack of proper nutrition can be easily identified. Ultimately, this means better quality care for the patient and greater peace of mind for the caregiver.

Physical Therapy

For patients who are experiencing decreased mobility due to an illness, injury, stroke or orthopedic surgery, VNA’s physical therapists develop an individualized plan of care, working with the patient to improve strength, endurance and ambulation.

Our therapists are also trained to identify factors in the patient’s environment or behavior, such as potential safety hazards or lack of proper nutrition, that would be detrimental to the rehabilitation process. Ultimately, this means better quality care for the patient and greater peace of mind for the caregiver.

Occupational Therapy

Occupational therapy is designed for patients who need assistance with daily activities after a hospital stay, injury or illness. VNA’s occupational therapists develop individualized plans of care for patients to help them develop, build or regain the fine motor skills needed to be able to independently perform activities such as eating, dressing and bathing in order to safely continue living in their own home.

Low Vision Works

Low Vision Works is a home-based occupational therapy program that involves visual retraining and environmental manipulaiton that has been shown to not only significantly increase visual ability, but also improve the quality of lives.  An Occupational Therapist specifically trained in the Low Vision Works rehabilitative process, over the course of 7-8 weeks, will assist in improving independence.  Those who would be successful candidates have problems, even with glasses, with recognizing faces of family and friends, watching TV, reading smaller print, seeing steps or curbs, writing checks, personal grooming, and cooking and have been diagnosed with Wet or Dry Macular Degeneration, Diabetic Retinopathy, Retinitis Pigmentosa, Glaucoma, or have Stroke-related vision loss.

Speech Therapy

Sometimes after a stroke or other injury or illness, patients may have difficulty swallowing or speaking. VNA offers the services of licensed speech pathologists who will conduct an initial assessment of the patient’s condition and develop a plan of therapy tailored to their individual needs, with the goal of helping the patient return to the level of functioning they enjoyed prior to suffering the injury or illness. Our speech therapists work closely with the patient to guide them through exercises and activities designed to strengthen the muscles of the mouth, neck and face in order to help them regain the ability to speak clearly, chew and swallow.
