VNA Hospice Services - Hospice in the Home

When a life-limiting illness no longer responds to curative treatment or the patient chooses to discontinue such treatment, VNA offers hospice care focused on providing comfort, hope and support for patients while allowing them to spend their final days at home.

Our skilled nurses and specially trained volunteers work together to provide the best physical, emotional and spiritual care, not only for the patient but for their loved ones as well.

Our interdisciplinary team of oncology clinical nurse specialists, hospice nurses, social workers,counselors and home health aides collaborate with physicians to implement a personalized plan of care to meet their specific needs that includes:

In-home hospice visits are scheduled to meet the ever-changing needs of the patient and their family. Between visits, hospice nurses are on-call 24 hours a day, seven days a week to assist with questions and concerns.

In addition, our trained volunteers provide companionship and compassion to patients, allowing caregivers time to attend to errands, work or other family members.

Our hospice representative will meet with you to review the patient’s needs and work with their physician for certification into our program. We will also examine your insurance options, notify you of coverage, complete insurance forms and bill the insurance provider directly.
